3. School's neighborhood (i.e. Cobble Hill, Carroll Gardens).
4. PTA's Check Contribution "payable to" info (legal PTA name and mailing address).
5. PTA's Contribution website URL.
6. PTA's email address.
7. PTA's website URL.
8. PTA's Annual Appeal letter URL (if applies).
